OFFICE HOURS
Our office, located at 3351 Masonic Drive [across the street from Cabrini Hospital, near the Alexandria Mall] is open from 8 am to 5 pm, Mondays through Thursdays, and from 8 am to noon on Fridays. Additionally, Mid State Orthopaedic & Sports Medicine Center provides a Saturday Morning Injury Clinic during football season, opening at 8:30 am to see any student athlete. FIRST VISIT
Your first appointment will allow us to gather vital information in treating your condition. Please bring with you:

  • Name and address of referring and family physicians
  • List of previous surgeries and other relevant medical records
  • List of current prescriptions and allergies to medications
  • Insurance information/claim forms and referral (if applicable)
  • All x-rays, scans, and pertinent test results
APPOINTMENTS
Routine appointments are scheduled in advance by calling during business hours. If you are unable to keep an appointment, reasonable notice should be given to the office. While every attempt is made to honor your appointment, emergencies may occasionally produce delays. We will try to notify you if a delay occurs to allow you to reschedule your appointment. To avoid any inconvenience, please be sure that our office has both your home and business telephone numbers in case we need to reach you.
  • ALL NEW PATIENTS should call our Nursing Department at 318-441-8325.
  • For returning patients, please call the front desk at 318-441-8307, 318-441-8306 or 318-441-8346.
INSURANCE
We participate in most major insurance plans. However, at this time we are unable to participate in Medicaid, Title XIX, DPW, or third-party liability accident cases. Patients are responsible for any deductible, co-insurance, co-payments and non-covered services. Mid State Orthopaedic and Sports Medicine Center is a specialty group. If you belong to a Managed Care Program, you may need a referral from your primary physician. If so, please bring a written referral form with you at the time of your visit.



BILLING
Payments for co-pays/deductibles/co-insurance percentages are requested at the time of each visit. Outstanding accounts are billed monthly. There is a nominal charge to be paid prior to the completion of disability forms. If you do not have insurance or require special payment arrangements, please call our office during normal business hours, and we will work with you to arrange a personalized payment plan. Our business office will help you with any questions about coverage.

EMERGENCIES, NIGHTS & WEEKENDS
A physician is on call 24 hours a day, seven days a week. In the event of an emergency, please call our office at 318-473-9556. After hours, the phone will be answered by an automated system that will page the doctor on call. You will be asked to leave your name, number and the acute nature of your problem. Your message will be relayed to a doctor, who will return your call. When in doubt, go to the Emergency Room of Christus Saint Frances Cabrini Hospital (Alexandria) and ask to be seen by a physician of Mid State Orthopaedic and Sports Medicine Center.

PRESCRIPTION POLICY
Prescription refills should be requested during office hours. Please plan ahead and call if you are running low on medications, as we do not accept refill requests during evening hours or weekends.

PRE-SURGERY PREPARATION
  • Do not eat or drink anything 8 hours prior to surgery, except approved medications with a sip of water
  • Wear loose fitting clothes
  • Arrange a ride home if you are having outpatient surgery
  • Fill your pain medication prescription before surgery
  • Notify your health plan and family physician in advance of the surgery
  • Hospital will notify you of arrival time on day of surgery

    Should you require hospitalization, our staff will assist in making arrangements for your hospitalization. If your insurance company requires a second surgical opinion or pre-certification, please alert our staff in order that the requirements of your insurer are met. You will be monitored each day of your hospitalization by a member of our orthopaedic medicine team.

    POST-SURGERY INFORMATION
  • Ice and elevate your extremity.
  • If your pain medicine causes nausea, you might get relief by taking the pain pill with food.
  • Plan to rest for a few days after surgery to minimize bleeding and swelling.
