Routine appointments are scheduled in advance by calling during business hours. If you are unable to keep an appointment, reasonable notice should be given to the office. While every attempt is made to honor your appointment, emergencies may occasionally produce delays. We will try to notify you if a delay occurs to allow you to reschedule your appointment. To avoid any inconvenience, please be sure that our office has both your home and business telephone numbers in case we need to reach you.
We participate in most major insurance plans. However, at this time we are unable to participate in Medicaid, Title XIX, DPW, or third-party liability accident cases. Patients are responsible for any deductible, co-insurance, co-payments and non-covered services. Mid State Orthopaedic and Sports Medicine Center is a specialty group. If you belong to a Managed Care Program, you may need a referral from your primary physician. If so, please bring a written referral form with you at the time of your visit. BILLING Payments for co-pays/deductibles/co-insurance percentages are requested at the time of each visit. Outstanding accounts are billed monthly. There is a nominal charge to be paid prior to the completion of disability forms. If you do not have insurance or require special payment arrangements, please call our office during normal business hours, and we will work with you to arrange a personalized payment plan. Our business office will help you with any questions about coverage. EMERGENCIES, NIGHTS & WEEKENDS A physician is on call 24 hours a day, seven days a week. In the event of an emergency, please call our office at 318-473-9556. After hours, the phone will be answered by an automated system that will page the doctor on call. You will be asked to leave your name, number and the acute nature of your problem. Your message will be relayed to a doctor, who will return your call. When in doubt, go to the Emergency Room of Christus Saint Frances Cabrini Hospital (Alexandria) and ask to be seen by a physician of Mid State Orthopaedic and Sports Medicine Center. TELEPHONE CALLS Our office staff is prepared to assist you. Non-urgent calls such as appointments, insurance problems or correspondence should be handled during normal business hours. Please plan ahead and call if you are running low on medications, as we do not accept refill requests during evening hours or weekends. PRE-SURGERY PREPARATION Should you have any questions, please contact Nursing at 318-441-8325 or 441-8327. Should you require hospitalization, our staff will assist in making arrangements for your hospitalization. If your insurance company requires a second surgical opinion or pre-certification, please alert our staff in order that the requirements of your insurer are met. You will be monitored each day of your hospitalization by a member of our orthopaedic medicine team.
